New Library form clarifications

Greetings all,


Today we begin using the new registration forms system-wide. You should have already received the forms via delivery. If you have not already done, please dispose of the old registration forms and use only the new form. If you have not received the new forms via Delivery, contact the Supply Room and ask that they be sent.

During the training sessions offered over the past three Fridays, several questions came up regarding how to handle some situations that staff anticipate will arise. Below are frequently asked questions and our suggestions for addressing the situations as they arise:

Q: What if a “second parent” comes in without the child’s card, and s/he wishes to gain access and check out?
A: In order for the “second parent” to have access, a new registration form that includes his/her name will have to be completed. The form may be completed and signed by the parent who wishes to have his/her information added to the account. In that case, the staff member would update the account to reflect the information on the new registration form, including any changes that might be needed for “responsible parent” and “second parent” information.

Q: Who is the “responsible parent” and who is the “second parent?”
A: The “responsible parent” is the parent/guardian who signs the registration form. His/her name should be in the “Parent 1” field on the registration form. The “second parent” is the parent whose name and information is in the “Parent 2” information field on the registration form.

Q: Does the “second parent” have to sign the registration form and be present at time of registration?
A: The “second parent” does not need to sign the registration form and need not be present at the time of registration.

Q: Can the “responsible parent” authorize adding “second parent” information to the account without completing a new registration form?
A: No. A new registration form must be completed to grant access to the additional parent/guardian.

Q: Do “responsible parent” and “second parent” information fields (name, ID and address) need to be added to juvenile accounts that were created before the information was required and are renewed?
A: Yes. Each new or renewed account should reflect the most current information and procedure.
